Reisaan Health

Your thyroid is being managed. You are not.

A 3-month program led by Dr. Roshani Sanghani — American Board-Certified in Endocrinology, Diabetes, and Metabolism. She investigates what is actually driving your symptoms, not just whether your TSH is in range.

Watch: Thyroid management with Dr. Roshani

Your TSH is normal. So why do you still feel like this?

You take the pill every morning. Your TSH comes back in range. Your doctor says everything is fine. Come back in six months.

But you are still exhausted. Still gaining weight. Still foggy.

Here is what nobody explained: the fatigue, the weight, the brain fog — they are not always coming from the thyroid. In most patients Dr. Roshani sees, these symptoms are driven primarily by insulin resistance. It looks almost identical to hypothyroid symptoms, travels alongside thyroid disease, and is almost never investigated in a standard thyroid workup.

That is why so many people remain symptomatic despite technically normal lab values.

If your TSH is managed but you still feel exhausted, foggy, or unable to lose weight — and nobody has looked beyond the thyroid number — this program investigates what else is driving your symptoms — whether you have hypothyroidism, Hashimoto's, or hyperthyroidism. The goal is not just a normal TSH. It is you actually feeling well.

Managing your TSH is not the same as improving the metabolic environment your thyroid lives in.

Thyroid function is sensitive to the lifestyle environment surrounding it. Dr. Roshani and her team review your full thyroid panel — TSH, T3, T4, TPO antibody — alongside your metabolic markers, insulin levels, and body composition. Then they address what they find in the right sequence: nutrition first, then sleep, stress management, and exercise. Each change improves the conditions for the next.

Thyroid medication has a clear role. The direction is always toward finding the minimum effective dose — not toward abrupt discontinuation. For patients in India, Dr. Roshani manages thyroid medications directly. For international patients, she provides clinical guidance for your local prescriber.

Your food. Your kitchen. Your life.

Dr. Roshani and her team don't give you a diet chart. They help you understand how different foods affect your thyroid and your metabolic health — and they work with the food you actually eat. Vegetarian, non-vegetarian, Jain, eggetarian, vegan. The most common reason people leave other programs is they could not sustain the diet.

What patients with thyroid conditions say

These are real, individual results — and they vary from person to person. Thyroid medication is only ever changed under medical supervision, never on your own.

Off the pill

In 3 months

“When I joined I was on a Thyroid pill but in 3 months it's well under control without the pill. The intrinsic values and confidence that I have gained is above and beyond. I feel more confident taking on any challenges in life — including motherhood, which has always been very scary for me.”

MansiHypothyroidism
1 month

No medication

“I was suffering from thyroid and tried all possible doctors — could not cure it. One month of strictly following and changing the lifestyle. It actually helped and it was a miracle for even the best of doctors, how it changed without any medication.”

ShriramThyroid
TSH 6.9 → 5.6

No thyroxine prescribed

“My TSH was 6.9. After just one visit with doctor and no thyroxine (which 99% doctors prescribe with this level of TSH), in 6 months my TSH is 5.6. I feel healthier and stronger.”

AartiHypothyroidism
TSH improved

In 3 months

“My blood report showing high TSH levels came as a shock to me. I began eating healthy or what I thought was healthy and exercised for a whole month, but all in vain. I felt even more exhausted than before.

She had dealt with hypothyroidism herself and had been living free of 'the pill' for the past 8 years. I thought to myself, 'She knows her stuff, she's a walk-the-talk kind of person.' Over the next 3 months Dr. Roshani proved me right.

On a rare occasion, her answer was a straight 'I don't know, there isn't enough research on this.' Her honesty left me amazed.

Within 3 months my TSH levels came down and every other health parameter improved. At the end of my 3-month programme with Dr. Roshani I feel fully equipped to continue working on my health.”

The metabolic connection your doctor may not be looking for.

Insulin resistance affects Indians at lower body weights and younger ages than Western populations. It creates an inflammatory environment that worsens autoimmune thyroid conditions like Hashimoto's. When both are present — and in Indian patients, they frequently are — treating them in isolation misses the interaction.

Most of our patients are of Indian origin, living across 12+ countries. We welcome patients from other backgrounds who want this approach.

What happens in the First Consultation

Before the call, Dr. Roshani and her team review your thyroid reports — TSH, T3, T4, TPO antibody — alongside your metabolic markers, current medications, and the lifestyle questionnaire you fill through the Reisaan app. This is a clinical conversation. You leave with a clear picture of what is driving your symptoms — is it the thyroid, insulin resistance, or both?

Note: Working online with Dr. Roshani does not replace the need for physical examinations with your primary care provider.

Dr. Roshani Sanghani

American Board-Certified in Endocrinology, Diabetes, and Metabolism

Dr. Roshani Sanghani has practiced lifestyle-first endocrinology for over 20 years. She is American Board-Certified in Endocrinology, Diabetes, and Metabolism, and a MINT-Certified Motivational Interviewing Trainer. Thyroid conditions — hypothyroidism, Hashimoto's, hyperthyroidism — sit directly within her clinical core.

She also brings something most endocrinologists cannot: personal experience. Dr. Roshani has Hashimoto's hypothyroidism. Through sustained lifestyle change — specifically stress management — her thyroid function stabilized to the point where she has not needed a thyroid pill for over a decade. She shares this not to make a promise, but because her patients deserve to know what is possible when the thyroid's environment changes.

Frequently asked questions

Thyroid & Your Health

Hypothyroidism, Hashimoto's thyroiditis, and hyperthyroidism. The hormonal imbalance is different in each, but the lifestyle foundation that supports thyroid function is the same. Dr. Roshani manages the clinical and medication side; the program addresses the lifestyle environment that affects thyroid function across all of them.

Two things, often simultaneously. First: TSH within range does not mean thyroid function is optimal for you. The reference range is a population average. Second — and more important for most patients I see — the fatigue, brain fog, and difficulty losing weight are often driven by insulin resistance, not the thyroid. If the insulin picture is not addressed, thyroid medication alone will not produce the results you are hoping for.

Yes — and I say this as someone for whom lifestyle change made the difference. I have Hashimoto's hypothyroidism. Through stress management practice, my thyroid function stabilized to the point where I have not needed a thyroid pill for over a decade. This is not a promise that it will take everyone off medication. But the lifestyle environment the thyroid operates in genuinely affects thyroid function.

Your first step

You have seen how this works. If it makes sense for your thyroid and your situation, the first consultation is where it becomes specific to you.
