In 2013, I got pre-diabetes. I was an endocrinologist.

Years earlier, during my fellowship, I’d already seen the disconnect. I was prescribing medications on one side of a corridor.

On the other side was a diabetes education class — where I first learned that carbohydrate triggers the maximum insulin response. That connection changed how I practiced. But I hadn’t changed how I lived.

Instead of accepting the diagnosis, I changed my lifestyle. More than a decade later, I have no diabetes — same genes, different trajectory.

I was on thyroid medication for years too — I’ve been off it for over a decade. But fixing my food alone wouldn’t have been enough. The stress was driving my blood glucose up. The poor sleep was making the stress worse. Everything was connected.

Three things frustrate me about how metabolic health is treated.

We treat high blood glucose like a fever.

Glucose is high — add medicine. Blood pressure is high — add medicine. Cholesterol is high — add medicine. We manage numbers without asking: why is this number high? What lifestyle is driving this? What would it take to change it at the root?

Doctors have zero training in lifestyle medicine.

They can write “diet and exercise” on a letterhead. The conversation stops there. No guidance on what to eat. No support for the behavioral change that makes it sustainable. The advice is right. But there’s nobody to help you actually do it.

We ask people to change habits without helping them change who they are.

Go on a diet. Reduce your carbs. Exercise more. These are instructions. They are not change. When the diet ends — and it always ends — you go back to being whoever you were before. That’s not a failure. That’s what happens when nobody helps you change the way you see yourself. Yo-yo dieting is not a character flaw. It’s a design flaw in how we treat people.

These three gaps are why I created Reisaan Health. An endocrinologist-led, lifestyle-first programme. Detailed protocols for nutrition, sleep, stress, and exercise. And motivational interviewing — the coaching approach that helps patients find their own reasons to change, in their own words. That took a team.

My team reviews your lab reports and full health history before we speak — they spend 30 minutes or more on this before the call. So when we meet, I’m not collecting information. I’m connecting the dots. You leave with a clear picture of what’s driving your condition and what to do about it.

Both. I see patients in person in Mumbai and online by video from anywhere in the world. Most consultations are online — it works just as well. All you need is an internet connection. Calls are scheduled between 8:30 AM and 2:30 PM IST, Monday to Saturday.

Type 2 diabetes, pre-diabetes, weight management, PCOS, thyroid conditions, Type 1 diabetes, fatty liver, and metabolic syndrome. I also work with people who have no diagnosis yet but want to prevent one. If you’re not sure whether your situation fits, book a first consultation — I’ll tell you honestly.

I review every patient’s history before their first consultation. I lead all clinical decisions and manage all medication changes. Between consultations, you work closely with your assigned health coach, who tracks your progress and adjusts your plan. I join alternate consultations to review how things are moving.

Yes. I’ve coached patients across 12+ countries — including the US, UK, Australia, Canada, Singapore, UAE, Nigeria, and Kenya. For patients in India, I manage medication directly. For international patients, I provide clinical guidance that you can share with your local doctor (prescriber). Working with me online does not replace the need for physical examinations with your primary care provider.

Your endocrinologist may be excellent at managing your medications. Most endocrinologists are — that’s what we’re trained for. What most practices don’t have is the time or the infrastructure to work on the lifestyle side alongside medication. That’s what Reisaan adds. You don’t need to leave your current doctor to work with me.

It’s a coaching approach that helps you find your own reasons to change — instead of me telling you what to do. The idea is simple: if the change wasn’t your idea, it won’t last. My job is to help you get in touch with what you actually want for your health, and then support you in getting there. I trained under Dr. William Miller, who co-created this approach over 40 years ago.

Because stress and emotional health affect your hormones and your blood glucose directly. I saw this in my own life — stress was one of the things driving my pre-diabetes. Some patients need support that goes beyond nutrition and exercise. Bhairavi works with patients on the emotional layer that most medical teams don’t touch.

The book teaches the framework — the four areas that drive metabolic health and the order in which to change them. The programme applies that framework to your specific body, with lab work, medication management, a coaching team, and consultations every 10-12 days. The book is your companion. The programme is your clinical team.

Three months. You have a consultation with Dr. Roshani and her team every 10-12 days, plus a video course, app tracking, and chat support throughout. Some patients reach their clinical goals before the programme ends. Others continue with maintenance — that’s discussed at the end, not upfront.
